For your convenience our Phoenix dental office accepts cash, checks, Visa, MasterCard and debit cards. Payment is expected at the time services are performed.
Insurance
As a courtesy to you, we will process your insurance claims for you and do everything we can to maximize your insurance benefits. Please keep in mind that your dental insurance is a contract between you and the insurance company, and it is ultimately your responsibility to understand your plan benefits. As a result, insurance claims will be filed with "assignment of benefits" in your name and a reimbursement check for covered services will be mailed from your insurance company directly to you.
The fees charged for services rendered to those who are insured are the usual and customary fees charged to all our patients for similar services. Your policy may base its allowances on a fixed fee schedule, which may or may not coincide with our usual fees. You should be aware that different insurance companies vary greatly in the types of coverage available. Also, some companies take care of claims promptly while others delay payment for several months.

Please contact our office to inquire about developing a treatment plan tailored to your specific dental needs. Only after your course of treatment has been planned should you apply for financing. Doing so sooner could mean that the fee plan you negotiate will not cover your entire treatment. Once your treatment plan has been finalized, you may get financing through Care Credit.
